The question is, why did that happen?

I was cleaning the carbon off with a wire brush wheel and a drill, i was holding the piston head with a shop towel and the wheel grabbed the rag and flipped it out of my hand and it hit the floor.

Wow. A bunch of "never do that" in that one sentence.

Starting with the most obvious... Never anything loose fitting or dangly around rotating machinery. Glad you didn't lose a finger.

Good luck finding a replacement piston and getting the motor back together. And I'm no expert, but if you haven't yet used the wire brush on the other three pistons, I wouldn't do that either. Looking at the marks left on the piston from the wheel, it seems too aggressive.

Better with a strong plastic scraper to get carbon off. At least its worth a try. Better for not damaging the soft aluminium.

I agree—way to aggressive! I think a new piston is in order.

I think it would run, probably for a while if no burs. BUT for the cost I'd replace it for the piece of mind.

Last week I did a top end rebuild on an xj maxim engine. The old pistons were petty beat up and the #1 had a cut off skirt. I ordered a new set
from ebay but tested my cleaning methods on the old set first. The wire brush was too aggressive, so I tested a suggestion I got on this
forum. 50/50 Acetone and ATF with a brass wire bush. I substituted various grits of scotchbrite pads for the brass brush. I used a coarse
paint stripper pad on the tops and medium and then fine pads all over. After an overnight soak in ATF the pads cleaned each piston
    spotlessly in about ten minutes.

Please tell me you didn't use this method on your gasket mating surfaces already...
Same solution mentioned above and some NEW razor blades work best there, with finesse!
Scratches of any magnitude are your enemy ;)

I use one of those single edge razor scrapers, being careful to not nick the surface, then I clean the surface up with a rag with lacquer thinner and 400 grit
wet paper wrapped around a wood block that has at least one good flat side.
