I have an 82xj650rj from new and I change my own oil. I drain the main engine oil and replace the filter but I have never removed the middle gear drain plug to drain the oil from this area.
Question is how important is this for me to do? How much oil comes out? Am I doing the bike any harm by not doing this?

How important? Not important at all.
How much oil comes out? Maybe 4 oz (100ml).
Doing any harm? Absolutely not. Would probably inflict more damage trying to get to the stupid plug - especially if you need to start removing exhaust. Juice not worth the squeeze.

Bottom line, don't worry about it. You're good. Change the oil and enjoy your ride.

If your engine was really a mess, I would run a couple short flush cycles to clean the entire engine, including that middle gear area.
