Retaining bolt for the brake drum arm fell out “someone” didn’t install a cotter pin in the bolt after reassembling the brakes. It’s a special bolt that has a shoulder on it and a cotter pin hole at the end.
It’s available from your local Yamaha dealer and flat washer, lock washer and nut and of course cotter pin.

Do you see that one part that seems a bit bent, is that normal?

I think it's time to set riding aside and thoroughly inspect everything on the bike.

We have all the parts you need in stock, but can't help out with a new mechanic, you'll be on your own for that...........

If you had a "shop" do that and not a "friends shop" I would be hesitant to ever go back to them. You are lucky, could have been much worse. Glad you are okay and now to finding parts and making sure the inside of the rear hub is okay?

This mechanic has been great from the start, is it possible perhaps it was knocked off by something? He's more than double check my bike from the start and he's been working on it the past 4 years. I'm greatful to be alive! To be honest no one else in my area will even TOUCH my bike because of its age. Can I get a list of the parts I'll need?

Just ask @chacal he will get you all the right bits just show him the part number I posted and your picture. And The rod did not survive so if Len doesn’t have one I do.

There is no way that failure can occur from anything but neglegance on the part of the last person to work on it.
Everyone makes mistakes, but that one should never be made.
